The Center has switched to the secure online MRI Screening Form that can be filled on a computer or mobile device.
Within 24 hours of the MRI appointment, the participant, if her/his email is provided on the schedule, will receive an automated email containing a unique link of the Screening Form. The email will be copied to the organizer and other attendee if applicable. If the participant email is not included, the experimenter will need to forward the link to the participant. After the participant completes the form, experimenter will receive an automated email notice.
